Polish personnel security questionnaire – how to complete it

The personnel security questionnaire is the basis of every Polish vetting procedure. Its quality largely determines how long the procedure takes.
Structure and key rules
- seven parts: personal data, family, career history, security, health, finances, referees,
- basic vetting (CONFIDENTIAL) skips parts V–VII; extended vetting skips part VII unless TOP SECRET access is sought,
- complete it personally; foreigners attach a sworn translation,
- never leave blanks – write „nie wiem” (I don't know) with the reason,
- concealing information is in itself a ground for refusal; checks should be completed within 3 months.
